$60-67+/hour Position Overview We are seeking a Pharmacist to join the team at a Medical Center, focusing on Transitions of Care and Ambulatory Care. This is an excellent opportunity for a pharmacist who enjoys working directly with patients and helping ensure a smooth transition from the hospital to home. The pharmacist will work with patients prior to discharge to provide medication education, counseling, medication reconciliation, and refill coordination as part of a Meds-to-Beds/Transitions of Care program. The goal is to help patients better understand and adhere to their medication regimens after discharge and reduce barriers to accessing their medications. Responsibilities Provide medication education and counseling to patients prior to discharge Perform medication reconciliation and identify potential medication-related issues Coordinate prescription refills and medications prior to discharge Support the hospital's Meds-to-Beds/Transitions of Care program Communicate with patients, providers, nurses, and other members of the healthcare team Help ensure patients understand their medication regimens before returning home Provide ambulatory/outpatient pharmacy support as needed Maintain compliance with hospital and pharmacy policies and procedures Ideal Candidate PGY1-trained pharmacists are strongly preferred Pharmacists without residency experience will also be considered Approximately 2 years of current inpatient experience is preferred for non-PGY1 candidates Retail and outpatient pharmacists with strong patient counseling experience are encouraged to apply Strong communication and patient-care skills Ability to work collaboratively with an interdisciplinary healthcare team Spanish fluency is a plus Why Consider This Opportunity?
