Job Description

Category Pharmacy - Pharmacist Job Type Contract This pharmacist role specializes in Transitions of Care within an ambulatory care setting at a hospital under the Los Angeles County Department of Health System. The primary focus is on supporting patients prior to hospital discharge by providing medication education, counseling, and assisting with medication refills to promote adherence and improve health outcomes. This role is often referred to as a "meds to beds" program, ensuring patients leave the hospital well-informed about their medications.

Responsibilities:

Provide medication education and counseling to patients before discharge Facilitate timely medication refills to support adherence Collaborate with healthcare teams to ensure smooth transitions from hospital to home Participate in ambulatory care clinic support as needed

Qualifications/Desired Experience:

Pharmacist license in good standing New PGY1 graduates preferred, but non-PGY1 pharmacists will be considered Background in retail or outpatient pharmacy settings is welcomed Fluency in Spanish is a strong plus due to patient population needs

This is a contract opportunity with a well-established health system known for its ambulatory and hospital-based pharmacy services. The hiring approach is contract-to-hire, allowing candidates to start immediately while the County's permanent hiring process is underway, which can take several months.
