Category Pharmacy - Pharmacist Job Type Contract This opportunity is ideal for pharmacists interested in transitioning patients safely from hospital to home with a focus on Transitions of Care and Ambulatory Care services. The primary goal is to improve patient outcomes by providing thorough medication education, counseling, and medication refills before discharge—commonly known as a "meds to beds" program. This vital role helps ensure patients understand their medication regimens and are more likely to adhere to them, supporting healthier recoveries. Responsibilities include working closely with patients prior to discharge to review and counsel on their medications, coordinating with healthcare teams to enable seamless medication access, and participating in ambulatory care initiatives across a large health system. This contract position supports a prominent county health system known for a wide range of outpatient clinics and hospitals. Preferred candidates are recent PGY1 residents, but those without postgraduate residency training who have retail or outpatient pharmacy experience will also be considered. Fluency in Spanish is a valuable asset for enhancing communication and patient engagement.

Licensed Pharmacist with a valid California license Recent PGY1 residency preferred; open to non-PGY1 pharmacists with retail or outpatient pharmacy backgrounds Strong communication and patient counseling skills Spanish language fluency is a plus Experience or strong interest in ambulatory and transitions of care pharmacy This contract role often serves as a pathway to permanent employment within the county health system, as their permanent hiring process may take several months.
