This role focuses on delivering comprehensive pharmaceutical care to patients, collaborating with healthcare professionals, and ensuring safe and effective medication use in a clinical environment.
Key Responsibilities:
Review and manage outpatient prescriptions, ensuring accuracy and appropriateness. Provide patient education regarding medication therapies and potential side effects. Collaborate with physicians and healthcare teams to optimize medication regimens. Monitor patient responses to drug therapy and adjust treatments as necessary.
