Ashmore Connection is currently seeking a Clinical Pharmacist to support an acute care healthcare setting. The Clinical Pharmacist will provide comprehensive medication management, collaborate with physicians and healthcare teams, and help ensure safe, effective, evidence-based medication use.

Responsibilities:

Review and optimize medication therapy for acute care patients Collaborate with physicians, nurses, and other healthcare professionals Monitor patient medication regimens, drug interactions, dosing, and therapeutic outcomes Provide clinical recommendations and medication-related education Support medication safety and quality improvement initiatives Maintain accurate documentation in accordance with facility and regulatory requirements

Qualifications:
