Contract Key Responsibilities:

Complete thorough medication reconciliation, creating accurate home medication lists prior to admission. Provide discharge medication counseling to patients and families, ensuring understanding and adherence. Monitor drug therapies for contraindications, interactions, allergies, and proper dosing. Offer clinical consultation and medication recommendations to healthcare providers. Participate actively in antimicrobial stewardship programs, interpreting culture results and guiding treatment for infections such as UTI, pneumonia, and SSTI. Perform pharmacokinetic and anticoagulation dosing and monitoring. Support medication-use review, quality assurance, and drug utilization evaluation efforts. Maintain pharmacy operations including medication inventory management, sterile product preparation, medication order verification, controlled substance accountability, and compliance with regulations. Participate in staff education, new employee orientation, and promote continuous quality improvement. Uphold safety and compliance standards to ensure a secure patient-care environment.

Qualifications and Experience:

Active pharmacist license applicable to the assignment. Experience in hospital or inpatient pharmacy settings preferred. Proficiency with EPIC electronic health records system. Strong clinical knowledge and communication skills. Ability to work independently within an LTACH environment. Skills in medication reconciliation and discharge medication list development. Experience with antimicrobial stewardship, pharmacokinetic dosing, and anticoagulation management.
