Our multidisciplinary team provides compassionate, high-quality care where patients live. The Role As our Pharmacist, you'll support safe, effective medication management for home-based patients and work closely with physicians, nurse practitioners, and care coordinators to improve outcomes and reduce hospital readmissions. What You'll Do Review and reconcile medications, resolving interactions, duplications, and dosing issues Collaborate with care teams to optimize regimens Educate patients and caregivers on safe medication use Support hospital-to-home transitions and monitor high-risk medications Document in the EHR and contribute to medication safety initiatives What You'll Bring PharmD from an accredited institution Active IL pharmacist license 1-3+ years of clinical pharmacy experience preferred (home health, geriatrics, or transitional care a plus) Experience managing polypharmacy in elderly and chronic care populations Strong communication skills and comfort with EHR and telehealth tools
