Requirements:

Pharm D. Degree from an accredited School of Pharmacy. 5 years of retail pharmacy experience required. Pharmacy operations leadership experience preferred. Registered Pharmacist in the state of Indiana and must be in good standing with the Indiana Board of Pharmacy. Additional certifications may be required.
