Category Pharmacy - Pharmacist Job Type Contract To Hire Pharmacist - Medication Therapy Management | Contract to Hire | 13 Weeks | Onsite or Remote This opportunity is tailored for a licensed pharmacist with expertise in Medication Therapy Management, offering a 13-week contract to hire engagement with flexibility for onsite or remote work. The role requires holding an active Louisiana pharmacist license along with a minimum of five additional state licenses. Priority will be given to candidates licensed in Louisiana plus DC, North Dakota, New Hampshire, New Jersey, South Carolina, South Dakota, Utah, and Wyoming.
Core Responsibilities:
Prepare and dispense medications per physician orders following established protocols. Interpret and accurately transcribe verbal and written prescriptions into computerized patient medication profiles. Maintain comprehensive and precise pharmacy patient medication records. Dispense pharmaceuticals and controlled substances while ensuring adherence to legal requirements. Monitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness of dosage. Detect, report, and document suspected adverse drug reactions promptly and accurately. Provide clinical consultations, suggest cost-effective therapeutic alternatives, and communicate effectively with medical staff. Participate in quality improvement initiatives and medication use review activities. Supervise pharmacy support personnel and verify daily activities of pharmacy technicians. Organize work assignments to ensure timely pharmacy service delivery with minimal supervision. Conduct regular medication storage inspections and manage inventory by removing outdated or unusable drugs. Respond professionally to telephone and in-person inquiries.
Qualifications:
Current pharmacist license in Louisiana is mandatory. Licenses from at least five additional states, with a target of ten states or more within 18 months. Prior retail or hospital pharmacy experience.
