Job Description

Consultant Pharmacist (PACE Program) Mt. Pleasant, MI Full-Time | Weekdays $130K-$150K Overview We are seeking a compassionate and clinically strong Consultant Pharmacist to join our team in supporting a PACE (Program of All-Inclusive Care for the Elderly) program in Mt. Pleasant, MI. This role is ideal for a pharmacist passionate about geriatric care and interdisciplinary, patient-centered practice. As a key member of the care team, you will help optimize medication therapy and improve health outcomes for older adults with complex medical needs. What You'll Do Review and manage participant medication regimens Participate in interdisciplinary team (IDT) meetings and care planning Conduct medication reconciliations and chart reviews Identify and resolve medication-related issues Ensure compliance with CMS, state pharmacy laws, and PACE guidelines Educate participants, caregivers, and care team members
What We're Looking For:
