Overview We are seeking a highly skilled and dedicated Pharmacist to join our healthcare team. The ideal candidate will possess extensive clinical pharmacy experience, including LTC and critical care settings, and demonstrate a strong commitment to patient-centered care. This role involves providing comprehensive medication management, patient assessments, and ensuring the safe and effective use of pharmaceuticals across diverse clinical environments. The Pharmacist will utilize advanced pharmacy software and EMR/EHR systems to support accurate medication dispensing, inventory management, and documentation. Certification in immunizations, aseptic technique, and IV infusion procedures is required, along with a solid understanding of physiology, anatomy, and medical terminology. Responsibilities Dispense medications accurately using pharmacy software and ensure compliance with all legal and safety standards. Conduct thorough patient assessments to determine appropriate medication therapy plans. Manage inventory levels effectively, including ordering, stock rotation, and documentation to prevent shortages or overstocking. Prepare compounded medications following aseptic techniques and maintain sterile environments for IV infusions and other parenteral therapies. Administer immunizations and provide patient education regarding medication use, potential side effects, and health management strategies. Collaborate with healthcare professionals on medical management plans for acute care, pain management, pediatrics, and other specialized areas such as ICU or hospital settings. Support patient service initiatives by assisting with medication administration procedures and patient care activities. Maintain compliance with HIPAA regulations and ensure confidentiality of patient information across all systems including EMR/EHR platforms. Participate in continuous professional development activities to stay current with evolving pharmaceutical practices and regulations. Requirements Valid pharmacist license with current registration in the state of practice. Proven experience in LTC pharmacy, critical care, or hospital environments; pharmacy residency preferred. Demonstrated expertise in clinical pharmacy practices including patient assessment, medication therapy management, and immunizations. Proficiency with pharmacy software systems as well as EMR/EHR platforms for accurate documentation and communication. Hands-on experience with compounding medications, aseptic techniques, IV insertion, infusion preparation, and inventory management. Strong knowledge of physiology, anatomy, medical terminology, pain management protocols, and medication administration procedures. Excellent communication skills for effective patient service and interdisciplinary collaboration. Ability to work efficiently within HIPAA guidelines while managing multiple priorities in fast-paced settings such as acute care or hospital units. Prior hospital experience is highly desirable; familiarity with medical management systems is a plus.
