Key Responsibilities:

Qualifications and Desired Experience:

Pharmacist licensed to practice in the state. Immunization Certification required. CPR Certification mandatory. Doctor of Pharmacy (PharmD) or Medication Therapy Management (MTM) certification preferred. Proven experience in long term care pharmacy services is a strong advantage.
