Job Description

Position Overview:
This is a full-time clinical staff pharmacist position with a schedule of 40 hrs/wk; 4-5 days/wk. The role requires rotation of weekday shifts, weekends, and holidays, with a rotation of call responsibilities. The pharmacist will function as an integral member of the pharmacy team, providing medication management, order verification, and clinical services across the hospital setting. Current NE Pharmacy licensure required. Degree from accredited college of Pharmacy.
Primary Responsibilities:
Enter medication orders into the Paragon software system in a timely and accurate manner, ensuring correct patient, dose, route, and frequency. Maintain and monitor inventory levels; perform cycle counts, restocking, and expiration date management to ensure continuous availability of essential medications. Prepare non-sterile and sterile compounds, including chemotherapy preparations, following USP and institutional sterile compounding policies and safe handling guidelines. Dispense medications for the Hospice program and coordinate transition-of-care processes when patients are discharged or transferred. Perform clinical duties such as chart reviews, medication reconciliation, patient education at discharge, pharmacokinetic dosing, and renal dosing adjustments. Verify pharmaceuticals prepared and delivered by pharmacy technicians to ensure accuracy and patient safety.
Supervisory and Educational Duties:
Manage pharmacy technician staff as needed, including workflow assignment, competency assessment, and performance coaching. Precept pharmacy students and serve as a clinical preceptor, providing training, feedback, and mentorship during experiential rotations.
Qualifications and Preferences:
Current NE Pharmacy licensure is required. Degree from an accredited college of Pharmacy is required. Hospital pharmacy experience is preferred but not mandatory; candidates should demonstrate strong clinical judgment, excellent communication skills, and a commitment to safe medication use. The successful candidate will be detail-oriented, able to work collaboratively across disciplines, and proficient with electronic medical record and pharmacy information systems such as Paragon.
