The Unit Based Pharmacist is a patient-centered, unit-based model of care that integrates pharmacists with responsibility for both the clinical and the distributive activities of the pharmacy department. Pharmacists are actively engaged in medication management in coordination with physicians and the interdisciplinary team and exhibit a high degree of ownership and accountability for the medication-use process in critical care settings.
•PGY1 completion preferred.
Qualifications:
Education, Knowledge, Skills and Abilities Required:
Bachelor's Degree from an accredited college of Pharmacy. Possess strong interpersonal skills and work well at the team level, as well as independently. Excellent written and verbal communication skills. Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills and Abilities Preferred:
Doctoral Degree in Pharmacy. A minimum of one (1) year Pharmacist experience in a hospital setting.
Licenses and Certifications Required:
