Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A direct hire opportunity is available for an experienced Infusion/Clinical Pharmacist to join a dynamic home infusion care team located in Auburn, NY. This role is pivotal in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications, ensuring patients receive effective, safe, and personalized therapy in the comfort of their homes.
Responsibilities:
Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment adhering to all state and federal regulations. Assess laboratory values, calculate pharmacokinetic dosing (e.g., for vancomycin), and monitor Total Parenteral Nutrition (TPN) for optimal treatment adjustments. Provide thorough patient counseling and education on specialty medications and treatment plans. Train patients and caregivers on infusion pump usage, sterile techniques, and catheter maintenance. Collaborate effectively with prescribing physicians, home health nurses, and interdisciplinary teams to ensure seamless therapy management and reduce hospital readmissions. Ensure compliance with all relevant regulatory standards, including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA. Manage pharmacy inventory, including refrigerated products, and coordinate packing and shipping to patients' homes.
Qualifications:
Current New York pharmacist license in good standing. Minimum of 2 years of experience in infusion specialty pharmacy. Experience with IVIg therapy is a plus. Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations. Proficient with CareTend system or similar.
