Category Pharmacy - Clinical Pharmacist Job Type Direct Hire An opportunity is available for a Clinical Pharmacist specializing in infusion therapy, focusing on the preparation, dispensing, and monitoring of complex intravenous (IV) and subcutaneous medications. This role involves serving as a vital member of a multidisciplinary team, ensuring the safe and effective administration of specialized pharmaceuticals to patients in a home infusion setting.

Direct Hire Responsibilities:
Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment adhering to state and federal regulations. Assess laboratory values and calculate pharmacokinetic dosing, including vancomycin and Total Parenteral Nutrition (TPN), to optimize patient treatment. Provide detailed patient counseling and education on specialty medications, infusion pump usage, sterile technique, and catheter maintenance. Collaborate closely with prescribing physicians, home health nurses, intake, referral services, reimbursement, nursing, clinical services, and pharmacy operations to coordinate patient care and prevent hospital readmissions. Maintain compliance with regulatory standards including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA. Manage pharmacy inventory, including refrigerated items, and prepare medication shipments to ensure timely delivery.
Desired Experience:
Minimum of 2 years' experience in infusion specialty pharmacy. Experience with IVIg therapies considered a strong advantage. Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations. Proficient using the CareTend system. Current New York pharmacist license in good standing.
