Category Pharmacy - Staff Pharmacist - IV Certified Job Type Direct Hire A home infusion clinical pharmacist position is available for a direct hire opportunity with an immediate start. This role demands expertise in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications within a home infusion setting. The pharmacist will serve as a key member of a multidisciplinary team focused on providing exceptional patient care, ensuring medication safety, and fostering effective communication among healthcare providers and patients.
Schedule:
Monday through Friday, 8:00 AM - 4:30 PM Participate in on-call rotation
Responsibilities:
Prepare specialty pharmaceuticals and IV solutions in a sterile environment, adhering to state and federal guidelines. Monitor lab values, calculate pharmacokinetic dosing (such as vancomycin), and oversee Total Parenteral Nutrition (TPN) to optimize treatment plans. Educate patients and caregivers on infusion pump usage, sterile techniques, and catheter maintenance. Collaborate with prescribing physicians, home health nurses, and internal teams to ensure coordinated therapy management and reduce hospital readmissions. Maintain compliance with relevant regulations including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA. Manage pharmacy inventory, including refrigerated products, and oversee packaging and shipping for safe patient delivery.
Qualifications:
Minimum of 2 years' experience in infusion specialty pharmacy IVIg experience is a plus Comprehensive knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations Current and active NY pharmacist license in good standing Experience with CareTend system preferred
