Job Description
Job Overview As a Pharmacist in Charge, you will play a pivotal role in ensuring the safe and effective delivery of pharmaceutical care within our healthcare facility. You will lead a dynamic pharmacy team, oversee medication management, and ensure compliance with all regulatory standards. Your expertise will be essential in providing exceptional patient service and optimizing medication therapies for diverse patient populations. What you'll do Supervise pharmacy operations, ensuring adherence to all legal and ethical standards, including HIPAA regulations. Manage medication inventory, including procurement, storage, and compounding medications as needed. Conduct patient assessments to develop individualized medication plans that align with clinical guidelines. Collaborate with healthcare professionals to implement medical management strategies for acute care and pain management. Provide immunizations and educate patients on medication administration and potential side effects. Utilize pharmacy software and electronic medical record (EMR) systems to document patient care activities accurately. Lead the pharmacy team in critical care settings, leveraging your clinical pharmacy experience to enhance patient outcomes. Mentor pharmacy technicians and interns, fostering a culture of continuous learning and professional growth. Requirements Doctor of Pharmacy (Pharm.
D.) degree from an accredited institution. Active pharmacist license in good standing with the state board of pharmacy. Experience in long-term care (LTC) pharmacy or hospital settings is highly desirable. Completion of a pharmacy residency program preferred; clinical pharmacy experience is a plus. Strong knowledge of medical terminology, physiology, anatomy, and aseptic techniques related to IV insertion and infusion. Proven ability to assess patient needs effectively and provide tailored pharmaceutical care. Familiarity with EHR systems and inventory management practices is essential. Excellent communication skills with a passion for delivering outstanding patient service.
