Category Pharmacy - Staff Pharmacist - IV Certified Job Type Direct Hire An infusion/clinical pharmacist is needed for a direct hire opportunity beginning ASAP. This role supports patients by preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications within a home infusion setting. Operating as a vital component of a multidisciplinary team, the pharmacist ensures safe sterile compounding, precise dose adjustments, and comprehensive patient education.
Schedule:
Monday to Friday, 8:00 AM to 4:30 PM Participate in an on-call rotation
Key Responsibilities:
Prepare specialty pharmaceuticals and IV solutions under sterile conditions adhering to state and federal regulations. Perform clinical monitoring by assessing lab values, calculating pharmacokinetic dosing (e.g., vancomycin), and managing Total Parenteral Nutrition (TPN). Provide detailed counseling and education to patients and caregivers about specialty medications, infusion pump usage, sterile techniques, and catheter maintenance. Collaborate closely with prescribing physicians, home health nurses, and internal teams including intake, referral services, reimbursement, nursing, clinical services, and pharmacy operations to optimize therapy management and prevent hospital readmissions. Ensure compliance with all applicable regulations including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA. Manage pharmacy inventory, including temperature-sensitive products, and oversee the packaging and shipment of medications and supplies to patients' homes.
Qualifications:
Minimum of 2 years of infusion specialty pharmacy experience IVIg experience is a plus Familiarity with specialty therapy payer requirements, reimbursement processes, and contractual obligations Current and good-standing NY pharmacist license Experience with CareTend system preferred
Location:
