Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A home infusion clinical pharmacist plays a vital role in managing complex intravenous (IV) and subcutaneous therapies for patients in a direct hire opportunity located in Seneca Falls, NY. This position requires a highly skilled professional to oversee sterile compounding, monitor medication therapies, and provide essential education to patients and caregivers within a multidisciplinary team setting.
Responsibilities:
Prepare specialty pharmaceuticals and IV solutions in a sterile environment adhering to all state and federal regulations. Assess lab values and perform pharmacokinetic dosing calculations, including vancomycin, plus monitor Total Parenteral Nutrition (TPN) therapy adjustments. Provide thorough patient counseling on specialty medications and treatment plans. Educate patients and caregivers on proper use of infusion pumps, sterile techniques, and catheter care at home. Collaborate with physicians, home health nurses, and clinical teams to coordinate care and reduce hospital readmission rates. Maintain compliance with regulatory bodies including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA. Manage pharmacy inventory, including refrigerated products, and prepare timely shipment of medications and supplies to patient residences.
Desired Experience:
Minimum of 2 years of infusion specialty pharmacy experience. Experience with IVIg is a distinct advantage. Knowledge of specialty therapy payer requirements, reimbursement procedures, and contract compliance. Proficiency in CareTend system. Current and good standing New York pharmacist license.
