Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A home infusion clinical pharmacist plays a vital role in managing and delivering complex intravenous (IV) and subcutaneous medications directly to patients' homes. This position requires strong expertise in sterile compounding, clinical monitoring, and patient education to ensure the highest level of care and safety. As the clinical pharmacist, you will serve as the central coordinator for a multidisciplinary team, facilitating seamless therapy management and improving patient outcomes in a home care setting.
Core Responsibilities:
Prepare specialty pharmaceuticals and IV solutions within a sterile, ventilated environment compliant with all state and federal regulations. Monitor lab values, calculate pharmacokinetic dosing (e.g., vancomycin), and manage Total Parenteral Nutrition (TPN) plans, modifying treatments as needed. Provide thorough counseling and education to patients and caregivers on infusion pumps, sterile techniques, and catheter maintenance. Collaborate with physicians, home health nurses, and internal teams including intake, referral services, reimbursement, clinical services, and pharmacy operations. Ensure full compliance with State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant standards. Oversee pharmacy inventory, including refrigerated products, and coordinate the safe and timely shipping of medications and supplies to patients.
Desired Experience:
Minimum of 2 years experience in infusion specialty pharmacy. IVIg experience considered a plus. Strong understanding of specialty therapy payer requirements, reimbursement processes, and contract obligations. Current and valid NY pharmacist license in good standing. Experience working with CareTend system preferred.
