Job Description

Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered. A direct hire opportunity is available for a Home Infusion Staff Pharmacist in Syracuse, NY, offering a focused role within a specialized care environment. This position operates Monday through Thursday from 8:00 AM to 5:00 PM, utilizing the CareTend system to support patient care at home.
Key Responsibilities:
Review infusion orders and patient therapies for accuracy and effectiveness. Participate actively in clinical rounds to collaborate on patient care. Perform sterile compounding and review Total Parenteral Nutrition (TPN). Maintain accurate patient records and individualized care plans. Ensure compliance with United States Pharmacopeia (USP) standards and cleanroom protocols. Provide training and ongoing support to pharmacy staff. Drive operational improvements to enhance service delivery.
Qualifications:
Must hold current licensure as a Registered Pharmacist in New York State. Minimum of two years' recent experience in an acute care setting; alternatively, one year of recent home infusion therapy experience is preferred. Strong expertise in intravenous admixture services and intravenous therapy. Ability to evaluate and monitor drug therapies effectively, considering the clinical conditions and total needs of home care patients, in collaboration with physicians, nurses, and other healthcare professionals.
