Job Description

Sigma Rehab seeks a compassionate Speech Language Pathologist to deliver evidence-based therapy to diverse patients in a rehab setting. You will evaluate speech, language, cognitive-communication, and swallowing disorders; develop and implement goal-driven treatment plans; and collaborate with an interdisciplinary team. Responsibilities include documentation, family education, and participation in care conferences. Responsibilities Evaluate speech, language, cognitive-communication, and swallowing disorders Develop individualized, evidence-based treatment plans and functional goals Provide one-on-one and group speech-language and swallowing therapy Document evaluations, goals, and progress in the EHR in a timely manner Educate patients and families on home programs and communication strategies Collaborate with an interdisciplinary rehab team on care plans and discharge needs Participate in care conferences and quality improvement initiatives Maintain regulatory and ethical standards for clinical practice Contribute to a supportive, people-centered, and innovative care environment Required Skills Speech and language assessment Treatment planning Rehabilitation therapy techniques Swallowing disorder management (dysphagia) Electronic health records (EHR) documentation Interdisciplinary care coordination Patient and family education Goal setting and outcome tracking AAC tools and strategies Regulatory and clinical compliance
