Category Pharmacy - Staff Pharmacist - IV Certified Job Type Direct Hire An immediate opportunity is available for a Home Infusion Clinical Pharmacist with expertise in sterile compounding, IV specialty medications, and patient-centered care. This direct hire role is designed for pharmacists with at least two years of infusion pharmacy experience who thrive in a clinical and collaborative environment.
Schedule and Location:
Monday through Friday, 8:30 AM to 5:00 PM Participation in on-call rotation Located in
Dublin, OH Key Responsibilities:
Prepare specialty pharmaceuticals and intravenous solutions in a sterile environment, adhering to all state and federal regulations. Monitor clinical parameters including lab values and pharmacokinetic dosing (e.g., vancomycin), adjusting therapies such as Total Parenteral Nutrition (TPN) accordingly. Provide detailed patient counseling and education on specialty medication regimens and infusion pump use, ensuring safe and effective home therapy. Collaborate with prescribing physicians, home health nurses, and multidisciplinary teams to coordinate care and avoid hospital readmissions. Maintain full compliance with regulatory requirements including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA standards. Manage pharmacy inventory, including refrigerated products, and prepare medications and supplies for timely home delivery. Utilize the CareTend system to support pharmacy operations and patient management.
Qualifications:
Active and current pharmacist license in Ohio, in good standing. Minimum two years' experience in infusion specialty pharmacy. IVIg therapy experience is a plus. Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations.
