Category Pharmacy - Pharmacist Job Type Contract Provide expert pharmacy care across multiple hospital locations in the Greater Cleveland, OH area as a PRN Pharmacist. This role requires a versatile professional capable of managing inpatient medication services on an as-needed basis, supporting quality patient outcomes through clinical expertise and effective communication. You will operate within a flexible PRN schedule during day shifts, traveling between assigned facilities to ensure consistent pharmacy coverage and patient care.

ASAP Key Responsibilities:
Conduct thorough medication reconciliation and establish accurate prior-to-admission medication lists. Provide discharge medication counseling to patients and families, enhancing medication adherence and understanding. Monitor drug therapies for contraindications, interactions, allergies, and appropriate dosing. Collaborate with physicians and healthcare providers by offering clinical consultation and medication recommendations. Engage in antimicrobial stewardship, including culture result interpretation and infection-related recommendations. Perform pharmacokinetic and anticoagulation dosing and monitoring. Support medication-use review, quality assurance, and Pharmacy & Therapeutics Committee initiatives. Oversee medication inventory management, order verification, and compliance with controlled substance regulations. Participate in staff orientation, training, and educational activities to promote professional development. Maintain compliance with federal, state, and facility pharmacy regulations and promote workplace safety.
Desired Qualifications:
Active Ohio Pharmacist license required. Hospital/inpatient pharmacy experience strongly preferred. PRN availability with flexibility to travel between multiple Cleveland-area facilities. Day shift availability. EPIC electronic health records experience preferred. Strong clinical, communication, and independent working skills. Experience in medication reconciliation, antimicrobial stewardship, pharmacokinetic and anticoagulation monitoring.
