Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A home infusion clinical pharmacist plays a critical role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications for patients receiving care at home. This professional serves as the central coordinator within a multidisciplinary team, ensuring safe sterile compounding, precise medication adjustments, and thorough education for patients and caregivers to manage infusion therapies effectively.

Direct Hire Responsibilities:
Prepare specialty pharmaceuticals and sterile IV solutions in compliance with state and federal sterile compounding regulations Monitor clinical parameters, including lab values and pharmacokinetic dosing for medications such as vancomycin, and manage Total Parenteral Nutrition (TPN) therapy adjustments Provide patient and caregiver education on infusion pump operation, sterile techniques, and catheter maintenance at home Collaborate closely with prescribing physicians, home health nurses, and internal teams including intake, reimbursement, clinical services, and pharmacy operations to optimize patient care and reduce hospital readmissions Ensure pharmacy compliance with regulatory requirements from State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA Manage pharmacy inventory, including refrigerated products, and coordinate safe and timely medication shipment to patients Utilize CareTend system for pharmacy operations and documentation
Qualifications/Experience:
Minimum of 2 years' experience in infusion specialty pharmacy IVIg therapy experience is highly desirable Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations Active Ohio pharmacist license in good standing This opportunity suits clinical pharmacists passionate about direct patient impact through specialized home infusion care.
