Category Pharmacy - Staff Pharmacist - IV Certified Job Type Direct Hire A full-time clinical pharmacist role is available to support home infusion therapy with a direct hire opportunity starting ASAP. This position requires a skillful pharmacist to prepare, dispense, and closely monitor complex intravenous (IV) and subcutaneous medications for patients receiving home infusion therapy.
Schedule:
Monday to Friday, 8:30 AM to 5:00 PM Participate in on-call rotation
Key Responsibilities:
Prepare specialty pharmaceuticals and IV solutions aseptically in compliance with state and federal regulations. Assess laboratory values and calculate pharmacokinetic dosing, including drugs like vancomycin, and manage Total Parenteral Nutrition (TPN). Provide comprehensive patient counseling and education on specialty medications, infusion pump use, sterile technique, and catheter care. Collaborate with physicians, nurses, and pharmacy teams to coordinate care and prevent hospital readmissions. Ensure regulatory compliance with State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards. Manage pharmacy inventory, including refrigerated items, and prepare medications and supplies for secure and timely home delivery.
Qualifications and Desired Experience:
Minimum of 2 years of infusion specialty pharmacy experience IVIg experience is a strong plus Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations Current and good standing Ohio pharmacist license Experience with the CareTend system preferred This direct hire position offers a rewarding opportunity to work in a specialized clinical setting where your expertise directly impacts patient care and outcomes.
