Duties:
- Dispense prescription medications to patients and provide medication counseling
- Collaborate with healthcare professionals to ensure safe and effective medication use
- Review and interpret prescriptions for accuracy and appropriateness
- Monitor patient medication therapy for potential drug interactions or adverse effects
- Provide clinical pharmacy services such as medication therapy management and disease state management
- Participate in pharmacy quality improvement initiatives
- Maintain accurate patient records and documentation
Qualifications:
- Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy
- Active pharmacist license in the state of employment
- Completion of a pharmacy residency program preferred
- Strong knowledge of acute care, pediatrics, hospital pharmacy, and critical care settings
- Familiarity with utilization management and pain management principles
- Proficiency in medical terminology and anatomy knowledge
- Excellent communication and interpersonal skills
- Attention to detail and ability to prioritize tasks effectively```
