Job Description
Overview We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a critical role in ensuring the safe and effective use of medications for our patients. This position requires a strong commitment to patient care, excellent communication skills, and the ability to work collaboratively within a multidisciplinary team. The Pharmacist will be responsible for managing medication therapies, providing patient education, and ensuring compliance with all regulatory standards. Duties Review and interpret physician orders to ensure accuracy and appropriateness of medication therapy. Dispense medications accurately while adhering to established protocols and regulations. Provide comprehensive patient care by counseling patients on medication usage, side effects, and interactions. Monitor patient progress and adjust medication regimens as necessary in collaboration with healthcare providers. Participate in IV infusion preparation and administration, ensuring adherence to aseptic techniques. Conduct medication reconciliation during patient admissions, transfers, and discharges. Maintain accurate records of medication administration and inventory management using pharmacy software. Stay updated on new medications, therapies, and clinical guidelines relevant to acute care settings. Experience Doctor of Pharmacy (Pharm.
D.) degree from an accredited institution is required. Completion of a pharmacy residency is preferred but not mandatory. Proficiency in pharmacy software systems for effective medication management. Strong knowledge of physiology and medical management principles related to pharmacotherapy. Experience with IV insertion techniques is highly desirable.
