Rockmed LTC is a locally owned and operated long-term care pharmacy in Edgerton, WI. We are seeking a qualified Pharmacist to join our mission of providing exceptional pharmaceutical care to long-term care customers. (doctors, nurses, LTC staff and administrators). This position offers excellent work/life balance with Monday-Friday hours, occasional (minimum of 1 day per month) Saturdays and Sundays and on-call shifts. Job Type:

Open to Full-Time or Part-Time positions! Schedule flexibility available with either four 10-hour shifts or five 8-hour shifts.

Position Summary:

The Pharmacist position provides clinical and technical pharmaceutical services in a long-term care environment. This position involves dispensing prescribed medications, conducting drug utilization reviews, overseeing technicians, and collaborating to ensure optimal patient care. The pharmacist ensures all activities comply with federal and state regulations while maintaining the highest standards of patient safety and care.

Position Duties:

Clinical Review & Drug Utilization

• Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Conduct Drug Utilization Review (DUR) on all prescriptions and review clinical edits to ensure safe medication therapy. Patient Care & Documentation
• Maintain comprehensive patient databases including current and past medical history, clinical and laboratory data, and concurrent medications. Document all communications with physicians, nurses, and healthcare team members in medical records. Medication Management
• Oversee accurate preparation, packaging, and dispensing of medications according to prescription labels and medication administration records. Oversee both manual and automated filling processes including blister packs, bottles, syringes and more. Regulatory Compliance
• Ensure all orders are filled in accordance with Federal and State Long Term Care Pharmacy regulations. Maintain controlled substances logs and implement policies for safe procurement, storage, distribution and disposal of medications. Clinical Expertise
• Provide drug-related consultation to internal team and external customers (Directors of Nursing, Administrators). Make therapy recommendations to physicians and facility staff as appropriate. Team Leadership
• While this position has no formal supervisory responsibilities, this role must provide guidance to internal technician staff on LTC procedures. Professional Interactions
• Engage in courteous and professional interactions with all internal and external customers while maintaining strict patient confidentiality and HIPAA compliance.

Experience/Education:

Bachelor's degree from an accredited School of Pharmacy or Pharmacy Doctorate required Current Wisconsin Pharmacist license required Previous experience in Long Term Care pharmacy preferred

Continually utilize visual acuity to read fine and/or small print, and to work off a computer screen for up to 10 hours/day Frequently sitting and/or standing for up to 10 hours/day Having a range of motion from kneeling, to bending over to raising arms above head to occasionally retrieve stock
