Contract Key Responsibilities:
Perform thorough medication reconciliation and develop accurate prior-to-admission home medication lists using multiple patient information sources. Provide discharge medication counseling to patients and families to promote safe transitions of care. Monitor patient drug therapy for contraindications, interactions, allergies, and appropriate medication/dose selection. Offer clinical consultations and medication recommendations to physicians and healthcare practitioners. Engage in antimicrobial stewardship, interpreting culture results and guiding treatment for infections such as UTI, pneumonia, and SSTI. Conduct pharmacokinetic and anticoagulation dosing and monitoring. Participate in medication-use review, quality assurance, drug utilization, and Pharmacy & Therapeutics Committee activities. Maintain medication inventory and ensure timely procurement, sterile preparation, proper dispensing, and controlled substance accountability. Execute medication storage inspections, expiration reviews, emergency medication management, and documentation compliance. Support staff development through training, education, and acting as a resource and role model. Adhere to safety policies, promote confidentiality, and contribute to a safe patient care environment.
Qualifications & Experience:
Active pharmacist license valid for the assignment. Hospital/inpatient pharmacy experience preferred. Proficiency with EPIC pharmacy systems. Strong clinical knowledge, communication skills, and ability to work independently in an LTACH environment. Experience in medication reconciliation, discharge medication lists, antimicrobial stewardship, pharmacokinetic dosing/monitoring, and anticoagulation dosing/monitoring.
