Pharmacy Medication Reconciliation Under the direct supervision of a licensed pharmacist, the medication reconciliation technician will interview patients and their caregivers in the emergency department as well as in patient care areas for the purpose of compiling an accurate medication history. The information will be entered into the patient s electronic health record by the technician and reconciled by the provider. This individual will play an integral role in helping to prevent medication errors caused by inaccurate or incomplete prescription information in a patient s chart. You must be compliant with hospital policies and procedures, and be able to interpret and comply with state and Federal regulations. This individual will provide exemplary service to our patients and demonstrate integrity, dependability and the ability to recognize each coworker s unique contributions to the York Hospital culture . Essential Duties and Responsibilities including but not limited to : Performs independently with respect to all essential technician duties, and be able to assist the Director with the development of this program. Helps to establish, coordinate, and maintain daily work functions as it relates to the collection of patient data; optimizes time efficiently and effectively. Gathers information for best possible medication list from various resources (patient interviews, physician offices, community pharmacies, historical encounters, extended care facilities, or other acute care facilities to ensure a complete and accurate entry into the hospitals electronic health record. Effectively communicates with other healthcare professionals to prioritize patient care, and identify patent specific needs. Accurately enter home medications into the patient s chart for utilization by the provider responsible for admitting the patient. Updates the medication list in a timely manner so it can be used as the primary source of information for patient admission medication orders. Communicate delays or difficulties in the reconciliation process to the appropriate disciplines. Directs any unresolved issues to the admitting provider or supervising pharmacist. Documents resolution of discrepancies in the medical record so that the attending physician can review and validate the reconciled medication list. Maintains courteous and effective interaction with colleagues and patients. Resolves workplace crisis and conflict in a calm, constructive and professional manner. Demonstrates a commitment to meeting and exceeding the needs of the nursing and medical staff as well as our patients. Maintains a clean, neat and professional work area at all times. Maintains focus on medication safety; offers suggestions to improve safety based on best practice (e.g. ISMP); documents medication errors. Participates in and successfully completes mandatory education for pharmacy technician recertification. Performs all other duties as needed or directed to meet the needs of the department.

SKILLS, KNOWLEDGE AND ABILITIES

Demonstrates the ability to accurately perform pharmaceutical calculations. Demonstrates knowledge of medical terminology and pharmaceutical abbreviations. Education:

High School Diploma/GED Licensure, Certification, Registration State of Maine Registered Pharmacy Technician License National pharmacy technician certification

Experience Requirements :

National Certification with three years experience in a combined hospital and retail setting; Paragon EMR trained individual preferred. Work history that reflects the ability to work in conjunction with pharmacy administration to development and manage this program.
