In this role, you will be responsible for assessing, developing, and implementing personalized treatment plans to improve patients' mobility, strength, and overall quality of life. You will work closely with a multidisciplinary team to deliver exceptional patient care across various settings, including outpatient clinics, inpatient units, and home health environments. Your expertise in physiotherapy and physiology will help facilitate optimal recovery and rehabilitation for diverse patient populations, including geriatrics, pediatrics, neurology patients, and those recovering from orthopedic or surgical procedures. Responsibilities Conduct comprehensive patient assessments to determine functional limitations and develop tailored treatment plans aligned with clinical documentation standards. Implement evidence-based physiotherapy techniques such as trigger point therapy, therapeutic exercises, manual therapy, and modalities to promote recovery. Collaborate effectively with healthcare professionals—including physicians, nurses, occupational therapists, and social workers—to coordinate care plans and discharge strategies. Manage electronic health records (EHR) systems accurately to document patient progress, treatment interventions, and discharge planning details. Provide patient education on injury prevention, home exercise programs, and community care options to support ongoing rehabilitation outside clinical settings. Assist in discharge planning by evaluating patient needs for home health services or outpatient rehabilitation programs. Engage with patients across various age groups—pediatrics, adults, geriatrics—and settings such as outpatient clinics, inpatient units, hospitals, or home health environments. Qualifications Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings. Strong knowledge of anatomy, physiology, medical terminology, and clinical documentation standards. Experience working with geriatric patients and understanding of age-specific care considerations. Familiarity with outpatient rehabilitation center operations and outpatient clinic workflows. Ability to develop comprehensive treatment plans that incorporate discharge planning and patient-centered goals. Proficiency in EMR (Electronic Medical Records) systems and healthcare documentation practices. Excellent client communication skills to educate patients effectively and foster a positive therapeutic relationship.
