The ideal candidate will be responsible for evaluating and treating patients with various physical conditions to enhance their mobility and overall quality of life. This role requires a strong understanding of anatomy, physiology, and medical terminology, as well as the ability to develop effective care plans tailored to individual patient needs across various settings including inpatient, outpatient, and home health. Duties Conduct comprehensive assessments of patients to determine their physical therapy needs. Develop and implement personalized treatment plans based on patient evaluations and goals. Utilize various therapeutic techniques including trigger point therapy, physiotherapy modalities, and orthopedic rehabilitation. Collaborate with healthcare professionals to ensure coordinated patient care and effective discharge planning. Educate patients and their families on treatment plans, exercises, and home care strategies. Maintain accurate documentation of patient progress using EMR systems. Provide care across multiple settings including acute care facilities, outpatient clinics, pediatrics, geriatrics, and home health environments. Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes. Skills Strong knowledge of anatomy and physiology relevant to physical therapy practices. Proficiency in medical terminology related to patient care and treatment procedures. Experience in discharge planning and developing comprehensive care plans for diverse patient populations. Familiarity with EMR systems for documentation and patient management. Ability to work effectively in various settings including inpatient hospitals, outpatient clinics, and home health environments. Experience with pediatric and geriatric populations is preferred. Knowledge of acute care practices and the ability to provide effective rehabilitation services in a hospital setting. Excellent communication skills for educating patients and collaborating with multidisciplinary teams.
