Job Description

PRN Physical Therapist (PT) - Home Health Make a Difference. One Patient at a Time. Join our team of compassionate healthcare professionals and enjoy the flexibility of PRN home health while helping patients regain independence in the comfort of their own homes. As a Home Health Physical Therapist, you'll develop individualized treatment plans that improve mobility, reduce pain, restore function, and enhance quality of life. What You'll Do Perform comprehensive physical therapy evaluations and establish individualized treatment plans. Develop and implement therapeutic interventions to improve mobility, strength, balance, and functional independence. Monitor patient progress, modify treatment plans as needed, and document outcomes. Educate patients, families, and caregivers on exercises, safety, and disease management. Collaborate with physicians, nurses, and the interdisciplinary care team to ensure coordinated patient care. Maintain timely, accurate, and compliant clinical documentation. Follow infection control practices and all agency policies and procedures. Promote exceptional patient care while supporting the mission and values of the agency. Qualifications Graduate of an accredited Physical Therapy program. Current Physical Therapist (PT) license in the state of practice. Current CPR certification. Valid driver's license, reliable transportation, and current auto insurance. One (1) year of home health experience preferred. HomeCare HomeBase experience preferred. Physical Requirements Ability to stand, walk, bend, kneel, squat, crawl, and sit for extended periods.
