What You'll Do Perform comprehensive physical therapy evaluations and establish individualized plans of care. Develop and implement treatment plans to improve mobility, strength, balance, and functional independence. Monitor patient progress and modify treatment plans to achieve optimal outcomes. Educate patients, families, and caregivers on exercises, safety, and disease management. Collaborate with physicians, nurses, and the interdisciplinary care team to coordinate patient care. Complete accurate and timely clinical documentation. Maintain compliance with infection control standards, agency policies, and regulatory requirements. Qualifications Graduate of an accredited Physical Therapy program. Current, unrestricted Physical Therapist (PT) license. Current CPR certification. Valid driver's license, reliable transportation, and current auto insurance. One (1) year of Home Health experience preferred. HomeCare HomeBase experience preferred. Physical Requirements Ability to stand, walk, bend, squat, kneel, climb, and sit for extended periods.
