What You'll Do As a Home Health Physical Therapist, you will: Perform comprehensive patient evaluations and establish individualized treatment plans. Develop and implement therapeutic interventions to improve mobility, strength, balance, and functional independence. Evaluate patient progress and modify treatment plans to achieve optimal outcomes. Educate patients and caregivers on exercises, safety, and disease management. Maintain accurate, timely documentation in accordance with company standards. Collaborate with physicians, nurses, and the interdisciplinary care team. Follow infection control and universal precaution guidelines. Deliver compassionate, high-quality care while promoting Choice Health At Home's mission and values. Qualifications Graduate of an accredited Physical Therapy program. Current Physical Therapist (PT) license. Minimum of one (1) year of home health experience preferred. Current CPR certification. Valid driver's license and reliable transportation with current auto insurance. HomeCare HomeBase experience preferred. Physical Requirements Ability to stand, walk, bend, kneel, squat, and sit for extended periods.
