Job Summary:

The Physical Therapist (PT) in a Skilled Nursing Facility (SNF) evaluates and treats residents with mobility, strength, balance, and functional limitations. The PT develops individualized rehabilitation plans to improve independence, prevent complications, and support safe mobility and discharge goals.

Key Duties & Responsibilities:

Perform comprehensive physical therapy evaluations and functional assessments. Develop and implement individualized treatment plans. Provide therapeutic exercises to improve strength, ROM, balance, endurance, and mobility. Provide gait and transfer training using appropriate assistive devices. Treat patients recovering from surgery, illness, injury, falls, and functional decline. Provide neuromuscular re-education and functional mobility training. HAL01 Requirements Active Physical Therapist License Less than 1 year of Physical Therapy experience
