Job Summary Hencetek is currently seeking a dedicated and compassionate Physical Therapist (PT) to join a well-established Skilled Nursing Facility (SNF) in Live Oak, CA . In this role, you will provide comprehensive rehabilitation services to help patients improve mobility, restore function, manage pain, and achieve greater independence. Key Responsibilities Evaluate patients and develop individualized treatment plans based on their clinical needs. Provide physical therapy treatments in accordance with physician orders and established plans of care. Perform therapeutic exercises, gait training, balance activities, functional mobility training, and other appropriate interventions. Monitor patient progress and modify treatment plans as clinically appropriate. Educate patients and caregivers on exercises, mobility techniques, home programs, and injury prevention. Maintain accurate, timely, and compliant clinical documentation. Collaborate with nursing staff, physicians, PTAs, and other members of the interdisciplinary healthcare team. Ensure compliance with federal, state, and facility policies and regulations. Promote patient safety, dignity, and quality of care throughout the rehabilitation process. Qualifications Graduate of an accredited Physical Therapy (PT) program. Active California Physical Therapist License . Current CPR/BLS certification preferred. Previous Skilled Nursing Facility (SNF) or long-term care experience preferred but not required. Strong clinical, communication, and interpersonal skills. Compassionate, dependable, and patient-focused approach. Ability to work effectively as part of an interdisciplinary team. Why Join Hencetek?
