Physical Therapist Assistant (PTA) - Home Health Dignity Home Health is a locally owned and operated home health agency proudly serving Fresno, California, and the surrounding communities such as Merced . We are dedicated to delivering exceptional, patient-centered care with compassion, professionalism, and respect—all in the comfort of our patients' homes. Position Summary We are seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our growing team either Full Time or Per Diem in Merced . In this role, you will work under the supervision of a licensed Physical Therapist to provide high-quality care that helps patients improve mobility, restore function, and achieve their rehabilitation goals in a home health setting. Responsibilities Provide physical therapy treatments according to the established plan of care developed by the supervising Physical Therapist. Implement therapeutic interventions that promote patient safety, mobility, and functional independence. Monitor and document patient progress toward established treatment goals. Communicate changes in patient condition to the supervising Physical Therapist and assist in modifying treatment plans as needed. Educate patients and their families on exercises, safety techniques, and strategies tosupport continued progress at home. Regularly assess patient response to treatment and report clinical findings promptly. Assist with discharge planning and communicate when patients have achieved maximum therapeutic benefit. Complete accurate, timely, and compliant documentation in accordance withagency, state, and federal guidelines. Collaborate effectively with the interdisciplinary healthcare team to ensure coordinated, high-quality patient care. Qualifications Graduate of an accredited Physical Therapist Assistant (PTA) program. Current, active California Physical Therapist Assistant License . Excellent interpersonal, communication, and organizational skills. Ability to work independently and manage a flexible schedule. Home health experience is preferred but not required.
