Qualifications Required:
Current Physical Therapist license in State of CA Driver's License, Physical and Negative TB CPR certification Minimum 1 year of HH experience Able to travel to prospective patient's place of residence Able to stoop and bend; must be able to lift and transfer patients Cover Pleasanton, San Ramon and nearby citiesDesired :
Experience with a public health, home care, home health care, or hospital background Ability to demonstrate communication, presentation, interpersonal, analytical and problem-solving skills Bilingual Your Day-to-Day Activities The Physical Therapist (PT) is responsible for the delivery of rehabilitation services through coordination, implementation, and supervision of the patients.Duties include:
Evaluate Patients Needs, design a care plan, and begin implementation of that care plan Communicate with other members of patient care team and adjust care plan as needed Reach out for community services to assist patient as needed Physical treatments with patients as per designed care plans Review notes/charts of PTAs and sign off on their notes, communicating with them regularly Appropriate supervision of patients that have been handed to PTAs Collaborate with physicians, other agency staff and contract services to coordinate and implement the Plan of Care Documents direct patient services, coordination and collaboration with physician and other disciplines or provides care for the patient, ensuring document is complete, and complies with acceptable home health standards and Agency policy Reviews caseload charts documentation at least every 60 days to ensure compliance Assesses patient progress and effectiveness of care delivery in meeting patient goals Appropriate discharge planning !Job Type:
