Physical Therapist (PT)
Gateway Post Acute Pay Range:
$65.00-$70.00 per hour, based on experience Help Residents Move Forward—And Grow Your Career! Gateway Post Acute is seeking a compassionate and motivated Physical Therapist to join our rehabilitation team! This is an exciting opportunity to use your clinical expertise to help residents regain mobility, build confidence, and achieve greater independence. If you want rewarding work with a supportive team, we would love to hear from you! Key Responsibilities Evaluate residents' mobility, strength, balance, endurance, and functional abilities. Develop and implement individualized treatment plans based on assessments, physician orders, and resident goals. Provide therapeutic exercises, gait training, balance activities, and other appropriate interventions. Help residents improve transfers, mobility, and safe use of assistive devices. Monitor progress and adjust treatment plans to support meaningful outcomes. Educate residents, families, and caregivers on exercises, mobility techniques, and fall prevention. Maintain accurate and timely evaluations, treatment notes, progress reports, and discharge summaries. Collaborate with nursing, occupational therapy, speech therapy, and other interdisciplinary team members. Participate in care planning and discharge planning. Provide direction and supervision to Physical Therapist Assistants as appropriate. Follow facility policies, professional standards, and applicable requirements. Promote a positive therapy experience that respects each resident's dignity and preferences. Qualifications Graduate of an accredited Physical Therapy program. Current California Physical Therapist license in good standing. Current CPR/BLS certification. Previous skilled nursing or long-term care experience preferred. Strong clinical assessment, treatment planning, and documentation skills. Excellent communication, organization, and interpersonal skills. Ability to work independently and collaboratively.
