Job Overview Nurse Next Door is seeking a compassionate and motivated Physical Therapist (PT) to provide home health services to patients throughout the Sacramento region. The Physical Therapist will evaluate and treat patients in their homes to improve mobility, strength, balance, endurance, pain management, and overall functional independence. This is a 1099 independent contractor position offering flexible scheduling and competitive per-visit compensation. Essential Duties and Responsibilities Perform initial physical therapy evaluations and reassessments in patient homes Develop individualized treatment plans based on physician orders and patient goals Provide therapeutic interventions including: Gait and balance training Strengthening exercises Transfer and mobility training Fall prevention education Pain management techniques Range of motion and endurance training Evaluate home safety and recommend assistive devices or modifications Educate patients and caregivers on exercise programs and safe mobility techniques Monitor patient progress and modify treatment plans as appropriate Complete all documentation accurately and within required timelines Coordinate care with physicians, nurses, occupational therapists, speech therapists, and interdisciplinary team members Participate in case conferences and care planning as needed Maintain compliance with Medicare, HIPAA, California Department of Public Health (CDPH), and agency regulations Qualifications Graduate of an accredited Physical Therapy program Current and unrestricted California Physical Therapist license Current CPR/BLS certification Minimum one (1) year of clinical experience preferred Home health experience preferred Valid California driver's license Reliable transportation and proof of auto insurance Strong clinical, communication, and documentation skills Ability to work independently and manage patient caseload efficiently Independent Contractor Requirements Maintain active professional liability/malpractice insurance Responsible for all taxes and independent contractor obligations Maintain all required professional licenses and certifications Provide own transportation, phone, and basic work equipment Compensation Competitive per-visit compensation based on visit type and experience: Visit TypeCompensation Range Routine PT Visit$90
