Position:

Home Health Physical Therapist We are seeking a dedicated individual for a PRN, part-time, or per diem - based role as a home health Physical Therapist. In this position, you'll serve as a crucial partner to our patients, guiding them through the process of restoring mobility and ensuring optimal functionality. Your role will focus on helping individuals achieve their highest potential in terms of movement and quality of life across areas encompassing promotion, prevention, diagnosis, treatment, intervention, and rehabilitation.

Responsibilities:

Assess patients' physical conditions and limitations Develop personalized treatment plans and monitor regimen for progress Administer therapeutic exercises and modalities Educate patients and caregivers on exercises and techniques Collaborate with healthcare professionals for comprehensive care Maintain accurate documentation of patient care Recommend and instruct on assistive devices Adhere to ethical standards and patient confidentiality Provide effective communication with a supportive and caring atmosphere Empower patients to feel confident in their journey as they achieve vital rehabilitation milestones

Job Requirements:

Minimum 1 Year PT Experience Preferably at least 1 year Home Health experience (Training provided for PT without home health experience) Must have valid professional license to practice PT in the State of California Valid CPR License Valid Driver's License and your own reliable transportation
