Skyline Ridge Nursing & Rehabilitation Center seeks a dedicated Physical Therapist to provide high-quality, person-centered rehab services to our senior residents. In our collaborative, respect-driven culture, you will assess, plan, and deliver individualized treatment to improve mobility, function, and quality of life. Partner with an interdisciplinary team, educate residents and families, document progress, and contribute innovative ideas that support seniors' independence and overall well-being in our skilled nursing community. Responsibilities Evaluate residents' functional status, mobility, strength, and balance Develop and implement individualized PT treatment plans and goals Provide therapeutic exercises, gait training, and balance interventions Use modalities and manual therapy as appropriate and evidence-based Document evaluations, treatments, and progress in the medical record Collaborate with nursing, physicians, and rehab team on care plans Educate residents and families on home programs and safety Participate in care conferences and discharge planning Ensure compliance with regulatory and documentation standards Contribute to a respectful, collaborative, senior-focused culture Required Skills Geriatric physical therapy Patient assessment and care planning Therapeutic exercise prescription Gait and balance training Functional mobility training Manual therapy techniques Use of rehab modalities (e.g., e-stim, ultrasound) Electronic medical record (EMR) documentation Interdisciplinary team collaboration Patient and family education
