  • Full-Time Civita Care Center at Newington | Newington, CT $45
  • 50/hour | Full-Time | Benefits Post Rehab is seeking a Physical Therapist (PT) to join the rehabilitation team at Civita Care Center at Newington.
This role offers the opportunity to provide individualized therapy while helping residents improve mobility, restore physical function, and work toward greater independence in a skilled nursing and post-acute care setting. What You'll Do Perform comprehensive physical therapy evaluations and develop individualized plans of care Provide skilled interventions focused on strength, balance, gait, transfers, endurance, and mobility Establish functional goals and monitor resident progress throughout treatment Modify treatment plans based on clinical needs and response to therapy Educate residents and caregivers on exercises, safe mobility, and fall-prevention strategies Complete accurate and timely clinical documentation Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary care team What We Offer $45
