Physical Therapist Move for Life Physical Therapy-Home Care Therapist Move for Life Physical Therapy is currently seeking exceptional Physical Therapists to join our DE team servicing patients in their home or assisted living communities in the North Wilmington Area. Move for Life Physical Therapy is a privately owned and operated rehabilitation practice comprised of highly skilled physical, occupational and speech therapists that provide unparalleled rehabilitative services to patients residing in Pennsylvania, Delaware, Maryland and Massachusetts. It is our mission to make a positive difference in the lives of each and every one of our patients by providing the highest level of care through the efforts of a dedicated, compassionate, professional therapy team. Responsibilities include:
- Identify and meet patients' goals and needs
- Offer effective treatments that help improve clients' motion and mobility
- Develop care plans using a variety of treatment techniques
- Create fitness- and wellness-oriented programs tailored to patients' specific needs
- Provide quality, personalized and evidence-based care and proven interventions
- Motivate patients during treatment in order to help them function optimally
- Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination
- Consult with other healthcare professionals
- Evaluate effects and monitor and communicate progress
- Document patient care services
Requirements:
- Valid license to practice physical therapy
- Graduate from an accredited physical therapy program
- Proven working experience as a licensed physical therapist
- Must demonstrate professional demeanor, work independently, and manage time efficiently
- Prior experience with electronic medical records (EMR) preferred
- Extensive education and clinical expertise
- Knowledge of current treatment practices
- Excellent interpersonal communication skills
- Ability to manage patients with different types of personalities
