Work Environment This role is onsite, primarily focusing on home care, which allows you to work directly with patients in their own environment. The dynamic nature of traveling to different residences offers a unique perspective on the challenges faced by our clients. Our therapists work Monday to Friday, with the added flexibility of customizing their own hours to accommodate personal commitments as well as patient needs. Key Responsibilities Conduct thorough evaluations and assessments of patients in a home care setting to develop individualized treatment plans aimed at restoring maximal function. Employ diverse treatment techniques, such as therapeutic exercises, manual therapy, and modalities to alleviate pain and enhance mobility. Regularly document treatment progress and adjust plans as necessary to ensure that each patient's therapeutic goals are achieved. Design and implement comprehensive exercise regimens that patients can follow in their daily routines, promoting independence and self-care. Provide guidance and education to patients, families, and caregivers on rehabilitation practices and the proper use of assistive devices to encourage safe mobility. Attend multidisciplinary team meetings to discuss patient progress and collaborate on optimal care strategies. Stay informed about the latest developments in physical therapy by attending workshops, participating in professional networks, and reviewing current literature. Patient Population / Client Focus As a Physical Therapist at Aspire Care, you will primarily serve patients recovering from injuries, surgeries, or chronic illnesses. This role encompasses working with a diverse range of patients, including children and seniors, each of whom presents unique challenges and opportunities for therapeutic intervention. The focus on home care means you will witness firsthand the impact of your work on patients' quality of life, making meaningful connections that facilitate recovery. Qualifications Bachelor's Degree in Physical Therapy from an accredited institution. Current State License in Physical Therapy. Excellent verbal and written communication skills to effectively interact with patients, families, and team members. Proficient computer skills for documentation and treatment planning. Demonstrated flexibility in adapting to varying patient needs and environments. Current CPR Certificate. A Doctorate in Physical Therapy (DPT) is highly desirable but not mandatory. Preferred Experience While a minimum of two years of experience is required, we encourage applications from both seasoned Physical Therapists and recent graduates.
