The ideal candidate must have home health experience and be proficient in completing OASIS assessments , including Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments, as applicable. Responsibilities Perform comprehensive PT evaluations and OASIS assessments in patients' homes. Complete PT Start of Care (SOC) visits when assigned. Develop individualized plans of care and establish measurable therapy goals. Provide skilled physical therapy services, including gait, balance, transfer, strengthening, mobility, and safety training. Assess fall risk, functional limitations, and home safety. Educate patients and caregivers on exercises, mobility, fall prevention, and use of assistive devices. Coordinate care with physicians, nurses, therapists, and other members of the interdisciplinary team. Complete all clinical and OASIS documentation accurately and within required timeframes. Report changes in patient condition and communicate effectively with the clinical team. Requirements Current and unrestricted Physical Therapist license . Home health experience required/preferred. OASIS experience and ability to independently complete

assessments required. Knowledge of Medicare home health documentation requirements. Excellent communication, assessment, and documentation skills. Reliable transportation. Ability to manage assigned patients independently and meet documentation deadlines.

Pay:

$70.00 - $90.00 per hour
