In this role, you will be responsible for assessing, developing, and implementing personalized treatment plans to improve patients' mobility, strength, and overall quality of life. You will work closely with a multidisciplinary team to deliver exceptional patient care across various settings, including outpatient clinics, inpatient facilities, and home health environments. Your expertise will help patients recover from injuries, surgeries, or manage chronic conditions through evidence-based physiotherapy techniques and compassionate support. Responsibilities Conduct comprehensive patient evaluations to determine functional limitations and develop tailored treatment plans aligned with clinical documentation standards. Implement therapeutic interventions such as manual therapy, trigger point therapy, and exercises to promote recovery and enhance mobility. Collaborate with healthcare professionals including physicians, nurses, and occupational therapists to coordinate care plans and ensure optimal patient outcomes. Manage discharge planning processes by preparing detailed reports and recommendations for ongoing care or community-based support. Maintain accurate electronic health records (EHR) and electronic medical records (EMR) systems to document patient progress, treatment details, and compliance with healthcare regulations. Provide patient education on injury prevention, rehabilitation exercises, and home care strategies to foster independence. Work with diverse patient populations including geriatrics, pediatrics, neurology cases, orthopedics, and patients in outpatient rehabilitation centers or hospital settings. Skills Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice. Proven experience in outpatient clinic settings, inpatient hospital environments, or home health services with a focus on patient care. Expertise in developing treatment plans that incorporate discharge planning and tailored rehabilitation strategies. Familiarity with outpatient rehabilitation center operations and outpatient experience requirements. Ability to effectively communicate with clients of all ages while demonstrating empathy and professionalism in client interactions. Knowledge of physiotherapy techniques including trigger point therapy and modalities used in inpatient or outpatient settings. Working understanding of healthcare workflows involving acute care experience, hospital experience, or outpatient clinic procedures.
